Awaken Mental Health Case Management, a division of TASC, Inc., is seeking case managers to join our growing team. Your role will involve working closely with adults with mental health disorders to assess their mental health needs and develop personalized goals and service plans to help them live independently. Responsibilities:
--- Conduct mental health assessments for clients in various locations, including homes and community centers.
--- Develop personalized service plans in collaboration with clients to address their mental health needs and goals.
--- Coordinate with healthcare providers, social services, and community resources to ensure comprehensive care for clients.
--- Maintain accurate and up-to-date documentation of client progress and treatment plans.
--- Maintain a productivity rate of at least 55%.

Qualifications:
--- 1-2 years' experience in mental health case management.
--- Bachelor's degree in psychology, social work, or related field.
--- Strong interpersonal communication skills.
--- Knowledge of mental health resources and services in the local area.
--- Valid driver's license and reliable personal vehicle.

About Company:
Treatment Access and Services Center (TASC) is a nonprofit drug and alcohol agency located in downtown Reading, Pennsylvania. TASC's mission is to provide drug and alcohol treatment as an alternative to incarceration for nonviolent criminal offenders.

As an intervention agency, TASC conducts substance use screenings, assessments, and treatment referrals for inmates at the Berks County Jail System, and emergency room patients at Tower Health/Reading Hospital as part of the Warm Hand-Off program. TASC provides urinalysis testing and works closely with the Berks County Treatment Court programs and the Adult Probation & Parole Office.

Finally. through its mobile Awaken Mental Health case management program, TASC provides support to adults with severe mental illness throughout Berks County. Together, through its many service units, the 60 employees working at TASC help thousands of individuals overcome the devastating effects of substance use and mental illness each year.
